Insurance & Billing

Updated insurance information must be given to us at the time of service. Please have your insurance cards ready when you check in.

If your insurance company requires a co-payment, please be prepared to make the payment at the time of service.

You may owe a balance after insurance due to deductibles, co-pays, co-insurance, or non-covered services. Patient responsibility must be paid within 30 days of receiving a statement.

It is important that you understand your benefits and your responsibilities for payment.

Payment Options

We accept cash, credit/debit cards, and personal checks.

Billing Questions

If you have questions about your bill, please call 402-488-7400, option 3. Please leave your name, phone number, date of birth, account number, and a brief description of your question.

Insurance

We accept most major health insurance plans. However, it is your responsibility to contact your insurance carrier to verify coverage and benefits.

Individual plans may vary, and coverage is constantly changing. Most insurance plans allow you to see an out-of-network medical provider, but benefits and costs may differ.

Patients may be financially responsible if the insurance company denies responsibility. Lincoln Family Medical Group may not accept new patients with all insurance plans.

Self-Pay Policy

We do accept patients who pay out of pocket. Our self-pay fee is broken into two parts. The full amount is due on the date of service.

  • New Patient: $250 (Effective 01/01/2026)
  • Established Patient: $175 (Effective 01/01/2026)

The second part is services performed during the appointment. Fees will be discussed with your provider and payment will be collected at the end.

No Surprises Act

Under the law, you have the right to receive a Good Faith Estimate explaining how much your medical care will cost.

For questions or more information, visit No Surprises Act or call our office at 402-488-7400.

Well Visit vs. Office Visit

What is a Well Visit? Preventive Care
  • Complete physical exam
  • Blood pressure, blood glucose, and cholesterol screening tests
  • Pelvic exams and pap smear
  • Mammograms
  • Prostate and colorectal cancer screenings
  • Sexually transmitted infection testing
  • Vaccination review and update
  • Developmental screenings
  • Evaluation of future risks
What is an Office Visit?
  • Diagnosing and monitoring specific medical conditions
  • Addressing medical concerns and treatment plans
  • Post-hospitalization discharge care
  • Medication refills
  • Specialist referrals
  • Testing/lab results
  • Addressing new or worsening symptoms
  • Depending on benefits, an office visit can result in additional costs

Why might I receive a bill after a Well Visit?

Many insurance plans cover routine preventive care at 100%. However, if new concerns, symptoms, medication changes, chronic condition management, referrals, testing, or other medical problems are addressed during the same visit, that portion of the visit may be billed separately as an office visit.

Depending on your insurance benefits, this may result in a copay, deductible, or coinsurance amount. To help avoid unexpected costs, please let us know when scheduling if your goal is to keep the visit focused on preventive care only.

Helpful examples: A routine physical, screening recommendations, and vaccine review are generally part of a Well Visit. Discussing knee pain, a rash, anxiety, diabetes, blood pressure, medication refills or changes, abnormal symptoms, referrals, or treatment plans may be considered an Office Visit and may be billed separately.
